Otherwise a signed referral from you with signature and provider number requesting they be seen through government program and that they have no medical contra-indications stopping them from being fitted with hearing aid is also acceptable.

We no longer need to use the old application forms (green forms) as clients are directed to attend the clinic of their choice or apply online and be sent voucher. Once entered on the system they will be issues with a voucher immediately rather than the previous 3 week wait.
